As an ENT specialist, I help families understand their children's language development and support you when medical causes are involved.

What means language development in children?

Language development refers to the acquisition of a first language. Step by step, children learn to understand and use sounds, words, and sentences themselves. A child's language development begins already in the womb and continues over the years. It is completely normal for children's language to develop at different rates. While some children speak their first words at ten months, others may not do so until the 15th month – both can be completely normal. However, children not only acquire the ability to form words and sentences, but they also learn to use language as a tool – to express their needs, build relationships, and understand the world. Language development is more than communication: it is fundamentally important for social and cognitive development, strengthens the bond between parent and child, and shapes everyday life – from playing to learning to interacting with others. From a medical perspective, language is an interplay of hearing, speaking, thinking, and social interaction. There are significant individual differences – some children speak very early, while others take longer, without this necessarily being a cause for concern.

Language Development: Overview of Phases and Milestones

Phase 0: Preliminary Phase (0–6 months)In the first months of life, children express themselves mainly through crying, laughing, and initial vocalizations. They already respond to voices and sounds and begin to lay the foundation for later language development.

Phase 1: Babbling Phase (6–12 months)Between six months and one year of age, children experiment with sounds and syllable chains like "ba-ba" or "ma-ma." They try out different pitches and playfully discover their voice.

Phase 2: One-Word Phase (12–18 months)From about one year old, children begin to speak their first words. These often stand for whole sentences or desires, such as "ball" meaning "I want to have the ball."

Phase 3: Two-Word Phase (18–24 months)During this time, children combine simple words, for example "Mom car." This is the beginning of grammar development, and their language becomes more structured.

Phase 4: Vocabulary Explosion (2–3 years) Between two and three years, vocabulary grows rapidly. Children start asking questions and form short sentences, making their communication significantly more varied.

Phase 5: Language Refinement (3–4 years) Grammar and sentence structure continue to develop. Children speak more clearly and can express their thoughts more distinctly.

Phase 6: Language Consolidation (4–6 years) In the preschool years, language becomes more established. Children form more complex sentences, tell small stories, and communicate confidently and in a nuanced way in everyday life.

A child's language development occurs in successive phases that correspond to certain age ranges. The timeframes are guidelines—individual deviations of a few weeks or months are completely normal and nothing to worry about. Each child goes through these phases at their own pace.

When should you become alert? Important information for parents

Parents know their child best and often notice early on when something develops differently than expected. It is especially worth paying close attention to language: Some irregularities are harmless, while others may be an indication that medical support is needed. Typical signs that should be taken seriously:<ul class="editor-list-ul"><li value="1" class="editor-listitem">Delayed speech development: When children start speaking words or sentences significantly later than their peers.</li><li value="2" class="editor-listitem">Noticeable or “atypical” sound formation: For example, strong lisping or sounds that are produced in an unusual way.</li><li value="3" class="editor-listitem">Unintelligible pronunciation: When children are difficult to understand even at kindergarten age.</li><li value="4" class="editor-listitem">Child does not respond reliably to speech: Frequent need to repeat or lack of response can indicate a hearing disorder.</li><li value="5" class="editor-listitem">Sudden regression in language: When children lose words or skills they previously had mastered.</li><li value="6" class="editor-listitem">Lack of eye contact or limited interest in language: When children barely respond, communicate little, or do not actively use language.</li><li value="7" class="editor-listitem">Accompanying symptoms: Recurrent runny nose, cough, or ear pain can also indicate a hearing impairment.</li></ul>

When to have an ENT examination?

Parents should seek medical support if the child's language development does not seem age-appropriate or if the mentioned signs appear. An ENT examination in children can clarify whether there are medical causes such as hearing impairments, anatomical peculiarities, or other factors. The earlier an evaluation takes place, the better children can be specifically supported.

Medical causes that can affect language development

In order for children to learn language well, they need not only curiosity and practice but also healthy physical conditions. Sometimes medical factors can make language development more difficult or delayed. These causes are often treatable – it is important to recognize them in time and provide targeted support. Common Causes:<ul class="editor-list-ul"><li value="1" class="editor-listitem">Hearing Disorders: Fluid in the middle ear, recurrent ear infections, or sensorineural hearing loss can affect hearing. Without good hearing, speech is impossible – children need to clearly perceive sounds in order to imitate and learn them.</li><li value="2" class="editor-listitem">Phonetic-Phonological Causes: A shortened lingual frenulum or atypical sound production can make articulation difficult.</li><li value="3" class="editor-listitem">Neurological Factors: Developmental delays or genetic peculiarities can influence speech development.</li><li value="4" class="editor-listitem">Anatomical Causes: Enlarged polyps or tonsils, as well as issues with oral motoric skills, can impair speech production.</li></ul> The good news: Many of these causes are treatable. A thorough ENT examination forms the basis for targeted therapy. The earlier a cause is identified, the better we can help your child reach their full language potential.

Medical Support for Language Development: How I Can Help

As an ENT specialist, it is particularly important to me to empathically support children and their parents on the path of language development. Language is a central part of a child's development – and sometimes medical support is needed to identify possible causes of delays or irregularities early and treat them appropriately. In a personal conversation and through careful observation, I address each child individually. I make sure to assess language development in an age-appropriate manner. To comprehensively assess language development, holistic ENT diagnostics are my primary focus. This includes examinations such as:<ul class="editor-list-ul"><li value="1" class="editor-listitem">Hearing diagnostics: Hearing tests and tympanometry help to rule out or identify possible hearing impairments.</li><li value="2" class="editor-listitem">Speech diagnostic tests: As an ENT specialist, I can perform specific tests to evaluate language development.</li><li value="3" class="editor-listitem">Examination of the mouth, tongue, lips, and palate: Anatomical factors, such as a shortened lingual frenulum, can influence language development.</li><li value="4" class="editor-listitem">Assessment of neurological indications: If necessary, further examinations are conducted to exclude neurological causes.</li><li value="5" class="editor-listitem">Evaluation of age appropriateness: I assess whether the child’s language development corresponds to their age or whether additional support would be beneficial.</li></ul> If necessary, I offer targeted support to promote language development. This includes:<ul class="editor-list-ul"><li value="1" class="editor-listitem">Collaboration with speech therapists: If needed, I refer to experienced speech therapists and work closely with them.</li><li value="2" class="editor-listitem">Treatment of medical causes: Middle ear problems, a shortened lingual frenulum, or other medical factors can be treated to facilitate language development.</li><li value="3" class="editor-listitem">Individual therapy plan: Together with parents and professional colleagues, I coordinate a customized plan that ensures the best possible support for the child.</li></ul>

Promoting Language Development: What Parents Can Do

Promote through play: Children learn language best through play. Role-playing games, rhymes, finger games, or singing together are ideal ways to expand vocabulary.

Use everyday routines: Language can be wonderfully integrated into daily routines: naming ingredients while cooking, colors while dressing, or describing things you see on the way to kindergarten.

Reading aloud: Regularly reading aloud not only promotes language development but also imagination and concentration. Short picture books and rhymes encourage children to discover new word

Limit screen time: Television, tablets, and smartphones should be used in moderation. Direct communication with parents and other caregivers is far more valuable for young children's language development than passive media consumption.

Speak normally – no baby talk:According to speech therapy recommendations, parents should speak to their children in a completely normal way. Short, clear sentences have proven particularly effective, as they are easier for children to understand. Baby talk or overly simplified words are not necessary – children benefit from hearing real language and being able to imitate it.

Reduce pressure:Every child develops language at their own pace. Comparisons with other children help neither the parents nor the child. Instead of constantly correcting, parents can simply and calmly repeat the sentence correctly.

Support bilingualism:If children grow up with two languages, this is not a problem – on the contrary, it is an enrichment. It is completely normal for a child to initially prefer one language and only use the second actively later. What is important is to lovingly include both languages in everyday life, without pressure.

Learning a language is completely natural for children – they do it automatically and effortlessly in their everyday lives. The more relaxed and natural parents are about it, the easier and more relaxed the process will be for the child.
